Equality Health, a healthcare delivery system based in Phoenix, Arizona, has partnered with a grocer, Bashas’ Family of Stores’ Food City brand, to create a full-service health retail clinic.

Dubbed the Equality Health Walk-in Clinic, it is set to open on April 29 at a Food City location in Phoenix. The aim of the clinic is to bring accessible healthcare to underserved communities.

To address the social determinants of health, the clinic will provide primary care, behavioral health and social services support.

It will offer health and wellness services, and a pharmacy and on-site lab services will also be available. Patients can receive screenings for behavioral health needs including depression, substance abuse, suicide prevention and anxiety as well. Additionally, the care team at the clinic will guide patients to resources like transportation, housing and food programs; disability benefits information; and Medicaid and Medicare enrollment information.

“The personalized clinic model completely focuses on total health, embraces culture and empowers the health of individuals and communities,” Mark Wright, president of Equality Health Medical Group, said in a statement.

Going forward, Equality and Bashas’ intend to build additional clinics in communities where it’s challenging for individuals to get access to healthcare, wellness programs and affordable healthy food.

“Bashas’ Family of Stores is committed to supporting our communities and the customers we serve through access to fresh, quality food in many areas of our state where social determinants of health play a role in daily life,” Bashas’ Family of Stores president and CEO Edward Basha III said in a statement.

Other organizations are also focused on bringing healthcare services to underserved populations.

Cityblock Health, for instance, recently raised a $65 million Series B round to address the social determinants of health. The startup combines primary care services, behavioral care, chronic disease management, substance abuse treatment and services like housing, transportation, and access to healthy food.
