It can be extremely challenging for both patients and medical professionals to comprehensively manage or prevent COPD-related hospitalizations, particularly within high-risk populations. To address this issue, Epharmix developed EpxCOPD, a unique, evidence-based remote patient monitoring tool. Clinicians at Washington University School of Medicine and Saint Louis University School of Medicine implemented EpxCOPD, which is designed to track patients’ daily breathing status as part of efforts to minimize preventable hospitalizations.

EpxCOPD functions in two ways: The tool proactively sends automated text messages and phone calls specific to a patient’s medical condition to the patient’s own phone, and it collects and risk-stratifies patient-reported biometrics and symptoms, which are then categorized into actionable alerts sent to clinicians. This process allows for timely outreach that can in turn help prevent hospitalizations for COPD patients.

Both patients and clinicians found that Epharmix’s EpxCOPD made a difference in patient care. In fact, the tool helped prevent 1 hospitalization for every 9 patients using the tool. The study saw a 62 percent relevant reduction and 12 percent absolute reduction in the risk of a hospitalization related to COPD. EpxCOPD’s active monitoring and use of non-smartphone interventions creates a more accessible and thorough tool for COPD management that can help patients avoid hospitalization and help clinicians better care for their patients.
